Job summary

The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.

This role provides clinical leadership and professional expertise to manage a large team of clinical and non-clinical staff. You would be responsible for setting and maintaining nursing quality standards ensuring adherence to the National Framework for Continuing Healthcare and Funded Nursing Care, Children and Young People's Continuing Care and S117 guidance.

The post holder will oversee systematic monitoring and intelligence gathering ensuring the quality and safety of care provision and working with colleagues in social care to coordinate timely remedial action when required. They will be responsible for ensuring equity in the assessment of patients for Continuing Healthcare and Funded Nursing Care and Children and Young People's Continuing Care either via directly employed staff or through trained assessors in acute and other settings. The post works closely with the CHC commissioning, contracting, business and account management functions to ensure that strategic and operational decision making is underpinned by strong clinical leadership.

This includes responsible for development of clear quality thresholds for providers as part of a robust commercial quality strategy, with organisational policies and procedures through to implementation and performance management, in relation to customer requirements.

Main duties of the job

Key Responsibilities

  • Provide clinical leadership and expertise
  • Work with ICB quality leads to agree and deliver actions in response to poor provider performance.
  • Lead on the delivery of the All Age Continuing Care processes ensuring organisational clinical compliance with the National Framework for Continuing Health Care and Funded Nursing Care, Children and Young People's Continuing Care and S117 guidance.
  • Lead and manage highly complex, high-profile cases, acting as the health representative for legal proceedings.
  • Lead on development of Clinical Key Performance Indicators and Clinical Contract Schedules in line with customer requirements.
  • Provide specialist clinical advice regarding commercial market provision and interpretation of population needs analysis data to ensure commissioning managers are supported to commission appropriate and efficient services to meet the needs of the population.
  • Maintain oversight and management of the safeguarding process to ensure all safeguarding referrals are investigated in line with policy and framework within the service
  • Working with the Clinical Leads to ensure that the function is able to adapt and respond to realise opportunities to develop and grow the business.
  • Set stretching objectives and targets for the senior management team which deliver both contractual and developmental outcomes.
