To support and enable the delivery of the Nottinghamshire ICS Medicines Optimisation objectivesby:Developing networks and processes to ensure that the policies and decisions made by the AreaPrescribing Committee are successfully implemented across all healthcare sectors to enable fulldelivery of efficiency savings, patient safety and equity

Main duties of the job

Are you a forward thinking, experienced pharmacist who enjoys a challenge? We are looking for a confident and innovative new staff member to join our highly respected and well established Interface team within NHS Nottingham and Nottinghamshire. Working with stakeholders across all healthcare settings you will bring clinicians together to achieve our aims of an integrated, patient focused and cost effective medicines optimisation strategy. Supporting the Area Prescribing Committee to embed guidance and formularies, offering implementation support including identification of any blocks. You will link with specialists and patient groups to ensure policy updates are communicated, shared and consulted on appropriately.

About us

Managing complex queries and concerns around prescribing at the interface of primary and secondary care, community services and other provider organisations both locally and outside of area. In so doing, the post holder will have authority to make decisions on behalf of and act inrelation to the populations of the ICSWorking with acute trusts and community providers to resolve problems with transfer ofmedication responsibility across the interfaces of care settings.Monitoring and investigating prescribing trends outside of APC recommendations and to workacross the interface to resolve anomalies, bring prescribing into line and to ensure a consistenttransfer of appropriate medicines optimisation initiatives for patients moving between healthcaresettings

Optimise cost effective prescribing across the interface to ensure equitable access of medicines across the Nottinghamshire health community.

Facilitate the implementation of APC guidelines and formulary decisions working with all relevant clinicians across the health community to ensure efficient and timely uptake to safeguard the outcomes and efficiency savings predicted.

To work with commissioners to identify commissioning barriers which may block the effective implementation of Area Prescribing Committee outputs.
