The Population Health & Health Inequalities team has an exciting position to support and develop our shared approach to reducing health inequalities and ensuring preventative health care interventions across the patch.
Working collaboratively with our system partners including NHS, Local Authority and our community and voluntary sector the post holders will matrix work with a number of colleagues and other stakeholders, including region and national, to advance our ambitions to improve population health.
This will include focus on the Core20PLUS5 initiative, NHS Long Term Plan, NHS Inclusion Health Framework and other key drivers.

Main duties of the job
The post sits within the Population Health and Health Inequalities Team, ensuring the delivery of exceptional quality healthcare for all HIOW residents through equitable access, excellent experience and optimal outcomes.
The team is responsible for developing and driving and effective an strategy and delivery model, promoting innovation across the system and ensuring alignment with partners and place.
The ICB statutory duty to reduce inequalities for those accessing health services and between patients with respect to the outcomes achieved for them by the provision of health services.
Action against the five high impact actions to reduce health inequalities, the Core20plus5 healthcare inequalities programme and the NHS England Inclusion Health Framework.
NHS action on prevention, paying regard to primary and secondary prevention including smoking, weight management, physical activity and alcohol.
Effective utilisation of the Local Authority Public Health Core Offer
Alignment with ICB provider and place plans and ICP partner plans
The role will specifically support the Senior Manager with contributing to programmes to deliver an improvement in the health and wellbeing and the reduction in health inequalities for the people in Hampshire and Isle of Wight.
The role will also provide support with the team's business support functions along to ensure the workstream of programmes is effectively planned and managed, resulting in successful delivery.
