Main duties of the job

Duties vary from team to team but mayinclude the following:

  • Provision of non-clinical support to the assessment, review and case management processes for NHS All Age Continuing Care (Children and Young People)
  • Responsible for supporting Senior Nurse Assessor/Practitioners (Band 6) and/or Nurse Assessor/Practitioners (Band 5) in their roles
  • Pre-population of Decision-Support Tool information under the supervision of Senior Nurse Assessor/Practitioners (Band 6) and/or Nurse Assessor/Practitioners (Band 5)
  • Preparation of Funding Request Forms under delegated authority
  • Completion of non-clinical case management tasks on behalf of Senior Nurse Assessor/Practitioners (Band 6) and/or Nurse Assessor/Practitioners (Band 5); for example (but not limited to) liaising with patients/families, conducting reconciliation of 1:1 provision, liaising with care agencies around staffing and shift fill, supporting completion of care and support plans and Personal Health Budgets, monitoring Extra Contractual Referrals, conducting customer service/package of care follow-up enquiries
  • Responsible for updating the care management systems with appropriate documentation
  • To deliver work in a timely manner ensuring adherence to National Framework timescales

  • Educated to NVQ 3 level in a relevant subject or equivalent level of qualification or significant equivalent previous proven experience.
  • Knowledge of NHS Continuing Health Care and Funded Nursing Care guidance/policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act, Mental Capacity Act)
  • Demonstrable experience in dealing with the public and dealing with sensitive and confidential information.
  • Working knowledge of IT systems.
  • Experience of the application of the Confidentiality and Data Protection Act.

Communication and Relationship Skills

  • Clear communicator with excellent telephone, writing, data entry and presentation skills: capable of constructing and delivering clear information/ instructions to staff and service users
  • Ability to work under pressure in a busy working environment and able to multi-task
  • Good interpersonal skills with the ability to deal sensitively with patients and their families/representatives.
  • Effective team working.
  • Ability to pull together comprehensive draft reports under supervision, and data and letters.
