Job summary

An exciting opportunity has arisen for an experienced and motivated registered professional to join the IOW delivery system, CHC & Placements Team. We are looking for an experienced individual who can demonstrate excellent assessment and analytical skills, with experience in NHS Continuing Healthcare whose primary focus will be to assist in the delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised July 2022). You will support the Operational Manager to deliver on all the areas of CHC, FNC and Personal Health Budgets therefore previous knowledge and experience in CHC/FNC is essential.

Main duties of the job

In particular, working alongside the other Clinical Leads within the team, you will be responsible for:

Supporting the Operational Manager to establish and maintain robust systems to implement current policy and guidance relating to the NHS Continuing Healthcare processes.

Complex case management for people with physical disability and mental health conditions that are eligible for NHS Continuing Healthcare.

The line management, supervision & support, of the Specialist Practitioners within the team.

Supporting the implementation of personal health budget agenda to our cohort of clients.

Supporting Adult Safeguarding processes, ensuring that care packages and commissioned services are safe.

To facilitate implementation of safe, effective and sustainable packages of care for individuals with complex needs and monitor and improve standards of care through clinical review.

Essential

  • Registered Health Professional. ENB 998/Mentorship level 2/3. Experience of line managing staff. Educated to degree/working towards a degree. Demonstrates excellent knowledge of NHS Continuing Healthcare Framework/ Funded Nursing Care Policy & surrounding issues (National & local). Demonstrates excellent knowledge of Clinical and Information Governance Frameworks. Good working knowledge of complaints procedure. Understanding of Confidentiality and Data Protection Act. National agendas and effect on health and social care practice e.g. National Health Service Frameworks. Participated in development of care standards, policies and services. Experience of working in an acute and/ or a community based setting.

Communication Skills

Essential

  • Clear communicator with excellent writing, data entry and presentation skills: capable of constructing and delivering clear information/ instructions to staff and service users. Ability to work under pressure in a busy working environment and able to multi-task. Effective team working. Contribute to the development and delivery of training programmes to all staff and support and mentor junior team members in the CHC appeals process including appeals and retrospective needs portrayals.
