We are excited to announce a new opportunity within our Hampshire and Isle of Wight All Age Continuing Care Mental Health and ABI Team. The All Age Continuing Care team is friendly and supportive spanning a wide range of people who work together to support each other, patients and the wider system. We support over 6,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, children's continuing care and other joint funding and oversight of a budget of £247M. Following the re-design of our services, we have Senior Clinical Assessor/Practitioner posts available in Hampshire. The role is community based with flexible working from a local office.

Main duties of the job

Participating in complex and detailed information analysis of specific projects/reports requiring high levels of concentration. This includes the ongoing review of enhanced care packages - evidence for ongoing requirements for clients entitled to S117 aftercare.

Experiencing in working with people who have diagnosis of a mental health condition and/or Autism is required. We offer in house training opportunities and Team Leaders who are non-case holding in order to mentor and develop our Clinical Assessors with complex cases, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team, comprising administration, brokerage, commissioners, Deprivation of Liberty experts and case practitioners who provide a wide range of expertise and support.

As a Senior Clinical Assessor/Practitioner) in Mental Health and ABI Team, you will be responsible for routinely screening and approving funding applications for clients who require packages of care or placements that are eligible or have S117 aftercare entitlement under the Mental Health Act 1983. They work closely with our Southern Health Care Coordinators and social services colleagues to ensure that commissioned care is safe, appropriate, and cost effective through ongoing review and assessment.

Knowledge, Training and Experience

Essential

  • Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE. including AMHP's and mental health social workers or significant relevant experience (LD/MH/LDAP teams only)
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
  • Experience of assessment of health and social care needs - care planning and coordination.
  • Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles)
  • Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team)
  • Ability to work effectively under stressful conditions in a rapidly changing environment.
  • Good knowledge of Mental Capacity Act and relevant legislation, for DOLS team
  • Experience in communications and stakeholder management.
  • Effective working knowledge of Microsoft software.
  • A good understanding of the health and social care environment and roles and responsibilities within it.
