Job summary
This post supports the Head of Operations and Quality for Learning Disability/Mental Health and Acquired Brain injury to provide the operational delivery of the National Learning Disability and/or Autism programme. The post holder will be responsible for the day to day operations and management of the LDAP team, ensuring the patient care is safe, personalised and achieves the best outcomes for the patient and/or their family.
Main duties of the job
- To ensure the delivery of a case management, review, assessment and admission avoidance process to the population of adults with a Learning Disability and/or Autism registered with a General Practitioner or an adult living in Hampshire and Isle of Wight but not registered with a GP anywhere.
- The post holder is responsible for providing operational management expertise to ensure the safe and effective delivery of service and value for money.
- To manage the Learning Disability and/or Autism programme team(LDAP) maximising use of staff resources and delivering a service to the required policies, procedures and standards demanded by the ICBs.
- The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
- To ensure the service is delivered with the patient user at the heart of the assessment processes and that admissions and discharges from hospital take place adhering to best practice and NHS England guidelines and pathways.
- Chair complex professionals meetings with providers, hospitals, families and patients to ensure that contentious issues can be resolved with the least restrictive option and with the best outcome for the person.

- Educated to masters level or equivalent level of experience of working at a senior level in NHS continuing healthcare speciality.
- Professional qualification in Health or Social care.
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or equivalent.
- Evidence of post qualifying and continuing professional development.
- Good understanding of the structure of both the NHS and Social Care, including commissioning and planning processes.
