Main duties of the job
You will be part of a team that works closely with colleagues from adult social care and the acute hospitals to support patient discharges and the community assessment of ongoing care and support needs as part of the discharge to assess (D2A) scheme. Community assessments are undertaken to determine ongoing health and care needs, ensure the right support is in place for the individual and asses for eligibility for Continuing Healthcare (CHC) funding.
You will also have the opportunity to develop complex case management and review skills as well as be part of a dynamic team that will give you the opportunity to help shape our service as both local and national agenda's evolve.

However, If you have recently returned to practice or you are a mental health practitioner with case management experience, we will be very pleased to hear from you. We are looking for someone with excellent assessment skills and the ability to prioritise and manage a wide ranging caseload. Knowledge and experience of both complex case management and Continuing Healthcare, whilst preferable, is not essential as full training and ongoing development will be provided.
