We are looking for an experienced and highly motivated Specialist Practitioner for our Children and Young People Complex Care services across Hampshire and the Isle of Wight (HIOW) to join our growing team.
This is an exciting time to join the service with opportunities to work with a wide range of stakeholder's and in partnership with other agencies.
We are an expanding team working Monday to Friday, office hours and no weekends or shifts.

Children and Young People (CYP) referred to the CYP Complex Care service will receive a high-quality service ensuring that the child, family, and carers are at the heart of a personalisation approach. We work closely with adult Continuing Healthcare to deliver the NHS All -age Continuing Care vision.
Main duties of the job
The successful candidate will be working in a collaborative approach to address the child and family's needs; whilst working closely with colleagues from Social Care and Special Education Needs to provide a holistic approach to the support provided.
Though working collaboratively with other key professionals, the successful candidate will need to be able to complete assessments as part of the CCG delivery of the NHS Continuing Care framework, for the children in a full and succinct manner. These assessments are then presented by the team member to a multi-agency panel where eligibility will be discussed and agreed or declined.
The successful candidate will be part of the clinical team where they will work independently managing their own case load, where they whilst ensuring that they fulfil the ethos of looking after the whole child. They will have an opportunity to build cross agency relationships.
