Hybrid Working
The
role of a Nurse Assessor is an opportunity to use your experience and knowledge
in nursing, to complete comprehensive assessments of patients, in a variety of
settings within North Kent. Our assessments are of a high standard and we are
proud of the work that we carry out.
We
are a very hard working but supportive and friendly team. The Nurse Assessors
have regular meetings with their peers and their managers to ultimately share
information, improve assessment skills and learn from each other. We also
support with regular training that improves and updates our knowledge and
skills.
We
currently work in a hybrid pattern of working, with patients assessments taking
place in their home environment wherever possible.
Our
office is in Chatham, the centre of Medway, which is central to the larger area
that we cover, and accessible from the main road and rail networks.

Main duties of the job
The role consists of
three aspects: triaging referrals, completing the comprehensive assessments and
reviewing the patients that have been assessed as eligible for NHS Continuing
Healthcare funding.
We are looking for
Registered Nurses Adult or Learning Disability who has excellent
communication, organisational and assessment skills, together with a sound knowledge
of the NHS Continuing Healthcare pathways and tools.
You will be travelling
to patients homes/care homes and/or working from home and in the office.
You will need to be
flexible to the various aspects of the role, have good negotiation skills and
the ability to work in a calm and professional manner.

Job responsibilities
This role requires specialist nursing expertise to
undertake and be responsible for completion of timely and comprehensive
assessments and to apply the relevant criteria to establish eligibility for
services and care.
The post holder will complete any relevant assessment
tools, for example the tools within the National Framework for NHS Continuing Healthcare
and NHS Funded Nursing Care, in conjunction with individual /family and
Multidisciplinary Team (MDT) members.
The post holder will:
Case manage individuals in receipt of NHS funded care
and support, reviewing and reassessing needs, the service provision and support
required
Review and reassess individuals eligibility for NHS
CHC, FNC and any other NHS funded placements
Review any service, and care, commissioned with
regards to quality and appropriateness to the individuals need, to ensure that
the provider is delivering the requirements of the contract
Complete retrospective reviews as required and
directed by team manager
Be confident in presenting cases and making
recommendations on eligibility and care/support required
Attend appeal/review panel meetings and present cases
as appropriate
Participate in case conferences as required. Ensure
accurate minutes and record of meetings are kept and distributed as appropriate
Participate in Best Interest Meetings as necessary
Ensure
accurate and open communication with a range of organizations and individuals
Keep
accurate and robust records at all times
