Kent and Medway are looking for CHC Practitioners to work across Kent and
Medway delivering an effective, equitable, high quality and patient centred
service delivery in line with the National Framework for NHS Continuing
Healthcare and NHS Funded Nursing care, revised 2022 (The National Framework).
You
will be assessing individuals health needs using robust assessment procedures
to determine if they have a primary healthcare need, while adhering to the NHS
National Framework
The successful candidate will need to be able to
demonstrate excellent written and verbal communication skills, able to work in
stressful situations and be able to prioritise and organise their own workloads
to meet deadlines.
Main duties of the job
In
line with The National Framework, you will be responsible for the management of
NHS Funded Nursing Care (FNC) related activity which includes the completion of
CHC Checklists and Decision Support Tool and FNC reviews, ensuring value for
money placements which meets the needs of the individual as well as identifying
opportunities for development and improvement to assist delivery of best
practice healthcare.
You will be a nurse or an allied health
professional, with experience in assessing individual care needs and be highly
motivated. You will have the ability to work independently as well as part of a
supportive team. You will have good interpersonal and patient assessment skills
and experience with safeguarding procedures. There will be extensive liaison
with patients, families, care providers and other professionals within the
health and social care sectors.

Be an integral member of the NHS Continuing and NHS Funded Nursing care team across Kent and Medway
- Be responsible for completion of timely and comprehensive assessments and reviews, applying the principles of the NF
- Complete the assessment tools from the NF, in conjunction with individual/family and Multidisciplinary Team (MDT)
- Review and reassess clients eligibility for NHS CHC and Funded Nursing Care (FNC)
- Review the CHC and FNC service and care commissioned in regard to quality and appropriateness to client need
- Manage enquiries by various stakeholders in relation to NHS CHC and FNC, and provide detailed
- Complete retrospective CHC reviews as required.
- Be able to undertake assessments of capacity and coordinate/complete Best Interest Decisions as required.
- Ensure accurate and open communication and co-ordination with a range of organisations and individuals
- Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally
- Keep accurate and robust records at all times.
