An exciting and rare opportunity has arisen in Kent & Medway to play a lead role for the ICB in the operational and strategic leadership of All Age Continuing Care. (AACC) in the North Kent Area.

This role will provide leadership and advice on all aspects of service delivery, development, quality, clinical effectiveness, value for money and patient safety, supporting the Deputy Director to develop and implement a clear vision and strategic goals for continuous service development and improvement.

The All Age Continuing Care service sits within NHS Kent and Medway Integrated Care Board (ICB) providing All Age Continuing Care services to the North Localities of Medway, Dartford, Gravesham, Swanley and Swale. Key aspects of the role include the leadership and management of a team of approx. 60 professional clinical and administration staff delivering the co-ordination of functions of Continuing Health Care Assessment, Funded Nursing Care, Childrens Continuing Care, MH Sec 117 and Neuro Rehab.

Main duties of the job

The post holder will be responsible for:

Overall day to day management of the service and effective running of the Team.

Chairing and presenting at Independent and Local Resolution Panels and contributing to IRPs independently.

Contributing to corporate and AACC policies, develops and reviews AACC standard operating procedures ensuring implementation as required.

Developing and maintaining qualitative and quantitative in line with national framework and in relation to national KPIs

To support the Mental Health & Section 117 workstreams working with the Head of Service for Mental Health

Ensuring ongoing development of AACCs dashboard reporting, risk and issues management

Liaising with all functions within the service and with stakeholders and partners across Kent and Medway Integrated Care System(ICS).

Work with the Business manager to compile the AACC business plan, establishing and ensuring management of all of AACCs business returns, including planning and workforce returns

Ensuring the AACC information management system is; fit for purpose, properly managed and is further developed in order to inform and respond to service needs and that of relevant partners.

Leading the coordination of recruitment, training and development activity across the service

Oversight and management of all aspects of the service budgets.

Financial and Physical Resources

The post holder will have budgetary responsibility for a function and the services provided within that function. This will include evaluating the value for money of new contracts, monitoring the performance of existing providers and ensuring that provider performance aligns, and complies, with contractual terms and conditions;

The postholder will be directly responsible for all budgets associated with the management of the function.
