Job summary
We are seeking a caring, driven and forward-thinking Patient Pathway Coordinator (PPC) who is committed to providing first class patient care in the Staffordshire and Stoke-on-Trent ICB Elective Referral Coordination Centre.
The PPC postholder will be office-based and responsible for processing urgent cancer referrals from GP practices (currently for North and Central Staffordshire with the potential to expand to the whole county).
This service ensures that all cancer referrals meet NICE guidance and contains all the necessary clinical information to allow UHNM NHS Trust cancer specialists to triage the referral as quickly as possible.
The role is complex and requires attention to detail, accuracy in reviewing and inputting of information and a significant level of concentration to ensure the referral process is completed efficiently.
This role supports the urgent cancer pathway for NICE and Faster Diagnostic Standards (FDS). Due to the nature of the referrals the PPC will be expected to have conversations which require compassion, discretion and confidentially.
The post holder will work under the leadership of the ICB Planned Care, Cancer and Diagnostics portfolio structure.
This role is an office-based role within the ICB (home working is not possible in the role but flexible working hours may be considered)
Location Elective Referral Coordination Centre in Smithfield 1, Hanley Stoke-on-Trent.
Main duties of the job
This service ensures that all cancer referrals meet NICE guidance standards and contain all the required information necessary for patient triage before the PPC progresses the referral along the cancer pathway.
The PPC review and checking process includes essential patient blood tests. Ensuring specific bloods have been ordered by GPs and that the results of these tests are available and valid, i.e., within specified time limits. PPCs will therefore liaise with Primary Care colleagues and hospital clinicians in Haematology and Biochemistry Laboratories.
Referral documentation, including the referral form, is thoroughly reviewed by the PPC. They will ensure that all mandatory fields are complete, if anything is missing, they will contact the referring practice to obtain the necessary information.
PPCs ensure that urgent cancer referrals conform to the requirements of the cancer pathway and make decisions concerning what appropriate action needs to be taken. They progress referrals through NHS e-Referral Service (e-RS) a national booking system and assess blood test results on NHS ICE system.

Travel on occasions to a Hub location with at least 1 days notice
New starters are expected to collect IT equipment, from offices in Stoke-on-Trent, and if based at home are required to have appropriate desk/chair/ relevant equipment in compliance with DSE regulations
Posts at Band 8c and above, will take part in the On-call rota, flexible over 7 days, supporting the System Control Centre, with full training provided.
