Job summary
Join our award-winning SHaR PCN as a Care Coordinator specialising in Safeguarding, where you'll make a real difference to the lives of our most vulnerable patients across Ryedale and South Hambleton. This pivotal role sits within our well-established Personalised Care Team, focusing on coordinating care for children, families, and vulnerable adults who need safeguarding support.
You'll work collaboratively with GPs, practice teams, and multi-disciplinary professionals to ensure timely, patient-centred care planning. This is an opportunity to enhance administrative processes for our most vulnerable patients whilst being the vital link between primary care, secondary services, and safeguarding agencies.
Working 37.5 hours per week on a permanent contract, you'll be part of an innovative PCN that's committed to personalised care and reducing health inequalities. Main duties of the job
- Care Plan Coordination: Develop, review and implement safeguarding care plans, ensuring they're shared appropriately with the multidisciplinary team and uploaded to patient records
- Communication Hub: Act as the single point of contact between GPs, families, social services, and other agencies involved in safeguarding cases
- Administrative Excellence: Complete initial reviews of patient notes and prepare child protection reports for safeguarding lead GPs to review and sign off
- Training Coordination: Support the PCN in developing and coordinating safeguarding training needs across all practices whilst maintaining accurate training records
- Relationship Building: Establish effective working relationships with the Young People and Families Coordinator, Social Prescribing Link Workers, and external safeguarding professionals
- Quality Assurance: Monitor care plan implementation, identify bottlenecks in service delivery, and provide feedback to improve patient journeys
- Meeting Participation: Attend quarterly safeguarding leads meetings hosted by ICB nurses and named GPs, contributing to service development and policy updates
- Travel flexibly across all seven SHaR PCN practices (Pickering, Kirkbymoorside, Helmsley, Terrington, Stillington, Tollerton, and Millfield) while based from Easingwold Health Centre. Deliver community-based support and maintain strong working relationships with practice teams across the network.
