With a particular focus on frailty the Pharmacy team at KCHFT pride themselves with providing a specialist medicines optimisation support and guidance to patients, carers, healthcare and social care staff employed by KCHFT and other organisations through collaborative partnership working. The joint post holder will be expected to support the service requirements as part of their role.Optimising a person's medicines is important to ensure a person is taking and continues to take their medicines as intended. It is a person-centred approach where shared decision making is an essential part of evidence-based medicine and needs to be system wide.
Main duties of the job
You will work on location at agreed Community Hospital sites across Kent and at GP Practices within West Kent ABC PCN, Maidstone and remotely as agreed.This is an evolving and forward thinking service and you will support the provision of safe and effective medicines optimisation services to cohorts of patients within these settings, with, where appropriate,opportunity to follow patients across the Primary Care Interface into their normal place of residence. These Medicines Optimisation Strategy goals include:
- Understanding and improving patient, carers and staff experience
- Providing evidenced based choice of medicines
- Ensuring medicines use is as safe as possible
The joint post holder will work within their clinical competencies using specialist pharmaceutical knowledge and skills to work with all stakeholders measuring and monitoring medicines optimisationoutcomes.to improve and achieve the best possible outcome from medicines for every patient.
The outcomes from the goals are:
- Patients take cost effective, evidenced based medicines safely
- Patients take only necessary medicines that they agree to take.
- Patients are empowered to manage their medications independently
- Carers, health and social care staff are empowered to support patients to optimise medicines use
- Patients are supported through effective use of their medicines to stay at home and have minimal hospital admissions and re-admissions.
