Job summary

We are seeking an experienced and motivated Band 7 PCN Lead Nurse to join our Community Nursing teams within the New Romney and Tenterden Primary Care Network (PCN). This senior role provides leadership across two community nursing teams, supporting high-quality care for patients in their own homes and strengthening the partnership between community services and the PCN. Working closely with the Operational Manager, you will contribute to quality, performance, workforce planning and financial oversight, while ensuring the smooth day-to-day running of the service.

The Community Nursing Service plays a key role in supporting GP practices and wider MDT partners, delivering personalised, holistic care to patients with complex needs. As PCN Lead, you will help shape how care is delivered locally, working collaboratively with GPs, specialist teams, social care, hospices and other providers to ensure an integrated, patient-centred approach. The role includes a strong coaching element, supporting teams to use data, analyse caseloads, identify themes and trends, and make informed professional decisions.

Main duties of the job

As Band 7 PCN Lead Nurse, you will provide visible leadership and senior clinical oversight across the New Romney and Tenterden community nursing teams. You will line-manage Band 6 Team Leaders, support proactive caseload management and ensure adherence to KPIs, contractual requirements and CQC standards. Your role will focus on coaching, developing and empowering staff, helping them build confidence in clinical decision-making, service improvement and collaborative working. While primarily managerial, you will also have opportunities to support occasional clinical visits where appropriate.

You will work closely with PCN partners to enhance the quality of patient care, respond to local population needs and contribute to the ongoing development of the community nursing pathway. Excellent communication skills are essential, as you will build strong relationships across organisational boundaries and champion high standards of care. You will be supported with further education and training, alongside a buddy system for local guidance.
