The Wellbeing Centre at Edenbridge Memorial Health Centre supports adults with long-term health conditions, dementia and age-related needs to stay well, independent and safe at home.
Working Monday to Friday within a friendly, multidisciplinary team aligned to local Primary Care Networks, you'll play an important role in delivering proactive, person-centred care to patients who benefit from regular monitoring and support.
Main duties of the job
As a Band 3 Healthcare Assistant, you'll help deliver personalised wellbeing interventions that follow best-practice principles. Your work will focus on prevention, early identification of deterioration and empowering patients to manage their own health more confidently. You'll support the team in providing:

Monthly observations and routine health checks

Basic education and information to help patients remain safe and independent at home

Health promotion and support for managing long-term conditions

Regular monitoring and onward referrals when required
You'll work alongside nurses, therapists, GPs and wider community teams to provide integrated care for people over 18. The environment is supportive, collaborative and centred around helping patients maintain their wellbeing and avoid unnecessary hospital admissions.
