We are looking for passionate and skilled Community Clinical Nurse Specialist to join our Thanet Community Nursing Team, to be based at our brand new £10million state-of-the-art Thanet Health Hub.Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. Main duties of the job

As a Community Clinical Nurse Specialist, you will use your clinical experience, expertise and advanced skills to provide leadership and effective caseload management, working closely with users of our services and other stakeholders.

You will proactively manage individuals on the caseload utilising assessment, management and prescribing skills whilst also providing senior leadership and expertise to support the management of the wider complex caseload.

This role will cross PCN boundaries.
