Job summary
The Consultant Practitioner for Home Treatment Service leads the Home Treatment Service alongside the SAS DoctorsWest Kent.
We are a community team made up of a variety of staff including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners and Health Care Assistants.
The Home Treatment Services have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust and Acute Trust Teams.

Main duties of the job
The post holder will be a clinical expert in Frailty and will fulfil the 4 domains of Advanced Clinical Practice:
1. Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence2. Leadership and consultancy providing expert professional advice to patients, carers and colleagues3. Development and provision of education and training to other staff and trainees4. Service development, evaluation, research and audit.
