We are looking for a skilled Band 6 Nurse to join our professional Transfer of Care Hub in West Kent.

The team support the wards with complex discharges (Pathways 2 & 3) and work closely with system partners to ensure that all discharge pathways are working effectively and with the patients and their loved ones at the heart of their discharge journey. How we work across the discharge.

Main duties of the job

Main duties and responsibilities:

o Work closely with health colleagues to support the complex discharges out of the acute hospital to ensure the level of service is appropriate to meet their needs

o Provide a holistic approach to interventions, working with the whole person, their family or system, building relationships and networks, enabling individuals, their families and carers to achieve sustainable good outcomes. Have a good understanding of KCC law and procedures or know how to access this information/advice if required.

o Support and, participate in, joint and integrated working with a range of teams including community safeguarding teams, partner organisations and other parties to offer a broad range of options to clients and carers to keep them safe and reduce risk. Join wash up discussion within health to encourage sharing of knowledge and information.

Support and help complete complex Mental Capacity Assessments, allow shadowing for learning to increase confidence and knowledge.

o Participate in complex multi-disciplinary and best interest meetings or discussions to ensure safe, effective and planned discharges.

o Create and maintain accurate, up to date and reliable data, information and records in line with information governance framework requirements, standards and best practice to ensure compliance with legislation.
