Job summary
The East Kent Community Cardiac Nursing Team provide a specialist service to adult patients with complex cardiac conditions across East Kent, primarily heart failure. As part of the highly specialist nursingteam, you will help support patients with chronic cardiac conditions that have been identified as having ongoing needs and would benefit from focused management.
Whilst patients are predominately seen in the clinic setting, we also see patients in their homes or digitally where clinically indicated. As part of your role, you will mainly see patient at home and you may be requiredto travel between localities. It is essential to hold a full UK driving licence, we do not provide a vehicle. You will undertake the assessment, delivery and evaluation of care required for this patient group in keeping withyour role and competencies, and under the supervision of the specialist nurses. Provide the necessary health education and advice to support the patient in manging their cardiac condition.

Main duties of the job
Role Specific Responsibilities:
- Provide high quality, evidence based, and individualised cardiac care to clients in the communitysetting, thereby enhancing quality of life for patients with cardiac disease.
- Assist the specialist nurses in the planning, coordination, and delivery of care to cardiac patients, supporting a holistic approach.
- Offer educational support to clients, carers and members of the multidisciplinary teams on all aspects of heart failure management.
- Work alongside the Specialist Nurses in group education sessions with the goal to empowering and motivating patients, carers, and families to self-manages and live a healthier lifestyle throughunderstanding their diagnosis, symptom recognition and treatments.
- Daily management a small caseload of patients across the locality, working independently without direct supervision.
- Carry out specific cardiac observations (such as blood pressure, ECG), phlebotomy and assessments (such as heart failure and fluid assessment). Support the ongoing management of cardiac patients, and accurately document finding and report results in a timely manner to the specialist nurse.
- Be aware of causes of heart failure, management, and associated comorbidities
- Undertake study and specific training as required to fulfil the role and work towards achieving appropriate competencies.
- Assess patients' compliance and safety with Heart failure medications and have a basic understanding of their effects and side-effect
