Job summary

Home First Service is our first move into joint working with NHS and KCC colleagues.

This new Service has been designed to offer a wrap round supported discharge from Hospital or to provide an integrated approach to prevent hospitalisation.Initially, the service will operate within the Thanet and Canterbury areas with a base at Westbrook House in Margate. Itwill operate 7 days a week, 8am to 8pm.

This is a new and exciting opportunity for a someone with strong Leadership and Management qualities to be at the start, steering the service in the right direction. Someone who would welcome the chance to create an excellent team andsupport the staff through any challenges. Someone able to monitor performance as the service progresses and makechanges to suit the needs of the service.

Main duties of the job

Our Enablement and Urgent Care services are responsive and flexible with the aim of supporting people to develop andmaintain their independence within their home environment.

Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day isessential. Car owner/driver essential (subject to the Equality Act 2010).

You should note that public transport is rarely a viable option as your role will involve travelling to rural areas, often with limited public transport.

We are looking for someone who:Believes in providing personalised support, building confidence and encouraging enablement.

Can work independently with confidence in their own decision-making skills.

Understands working to targets, budgets and reporting on both.

Wants to develop operational skills.

Has experience of working with a multi-skilled staff base and ensuring that these skills are used for the best outcomes.

Knowledge of working in the Health or Social Care sector is required.

QUALIFICATIONS

Essential

  • Holding or willingness to work towards Management qualification at Level 5, Level 5 Diploma for Health and Social Care or equivalent
  • Holding or willingness to work towards Qualified NVQ Assessor
  • If it is stated in your Contract of Employment that you should be registered with an appropriate professional body, e.g. GSCC, then this registration must be maintained at all times

EXPERIENCE

Essential

  • Background in Adult Social Care and or Health Care or previous first line supervisor/line management experience
  • Experience of supervising staff
  • Experience of working to contract specifications and delivery to required standards.

SKILLS AND ABILITIES

  • Ability to undertake and document formal risk assessments
  • Ability to communicate effectively with all levels of staff, People using the service and external contacts as required by the job
