Job summary

Urgent Care Services offers short-term health, therapy, and care support to help serviceusers remain at home. It treats acutely unwell individuals to prevent hospital admissions andaids hospital discharges, operating 24/7, 365 days a year.Night care includes end-of-life support, catheter care, and urgent unplanned care provided bya dedicated nursing team. Recovery at home is proven to be the best way to regainindependence, with enablement and rehabilitation as priorities.KCHFT is collaborating with East Kent Hospitals University Trust to develop a virtual ward forpeople with acute respiratory infections or conditions like COPD. Patients discharged earlywill be monitored at home via remote technology, such as apps, wearables, and medicaldevices, with support from consultants, specialist GPs, and advanced practitioners. Thespecific technology is still being developed with patient and colleague input.

Main duties of the job

As a core member of the Urgent Care Service, you will deliver essential clinical and non-clinical care, supporting patients in maintaining their independence and self-care.

Your role includes:

o Following care plans and accurately recording patient interactions

o Conducting basic observations (BP, pulse, oxygen saturations, temperature, NEWS2)

o Providing personal care, including washing, dressing, and mobility support

o Prompting medication administration and managing catheter and stoma care

o Assisting with fluid, nutrition, wound dressings, and subcutaneous injections

o Performing venepuncture, urinalysis, and specimen management

o Helping patients after falls using specialist equipment

o Supporting patients with blood glucose monitoring and risk assessments for skin integrity

Working within a multi-professional team, you will balance varied tasks and collaborate with registered practitioners to deliver high-quality, compassionate care. You will ensure patientsfeel respected and empowered by:

o Engaging families in care decisions

o Providingreassurance with confidence and professionalism

o Maintaining a kind, patient-focused approachTraining, development, and safeguarding remain central to the role. You will also contributeto mentoring new colleagues and continuously improving service delivery.
