Job summary
An exciting opportunity to join West Kent Urgent Care Services Team. Our Urgent Care Service provides a two-hour response to patientsin crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We alsoprovide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare.
We carry out detailed holistic assessments, developing personalised plans of care in collaboration with patients and their carers. Our patients are complex, and require proactive risk management, in order to be maintained safely at home. We maintain clinical oversight,and ensure all actions are completed appropriately.Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control andmanagement via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observationsand fundamental nursing care.This service is providing nursing care within patients' homes and covering West Kent. The ability to drive is essential to this post.

- Assessment, planning, implementation and evaluation of holistic careo The development of integrated care plans in partnership with the patiento To order equipment and send onward referrals as necessaryo To support in the supervision and development of all junior staff including Health Care Assistants and Studentso To actively work as a member of the multi-professional team to provide high quality care to patientso To undertake a range of competent clinical interventions and identify any deterioration in patients' condition, taking appropriateaction.o Manage a caseload of patients on a daily basis consisting of patients with complex needs associated within a specific speciality.o To support in duty role and complete triages
Additional Working Pattern: Full and part time hours available. Long days, weekends and night shifts
