Our work is practitioner-led and therapy focused, ensuring care is patient-centred.We will help you to develop your skills, and support your continued professional developmentas a valued colleague in one of the largest NHS trusts in the UK.This is a critical Occupational Therapy / Physiotherapy role in the delivering rehabilitation topatients residing in the Non-Weight Bearing (NWB) Pathway beds in Hawkinge House ProactiveAssessment Unit, Folkestone, East Kent. You will be responsible supporting recovery andrehabilitation, and working alongside the MDT to assess and plan discharges for patientsplaced on the NWB pathway.
Main duties of the job
The post holder will work within the Non Weight Bearing (NWB) Pathway supportingdischarges from acute hospitals in East Kent. These beds are situated within Hawkinge HouseProactive Assessment Unit, Folkestone, managed by the Community Assessment Bed team,where the post holder will be working as part of a Multi-Disciplinary Team providing therapyinterventions to support Recovery, Rehabilitation, Assessment and Discharge Planning forpatients recovering from fractures. The pathway incorporates 6 weeks of recovery andhealing, during which time therapy is provided to support recovery from non-weight to partialand full weight bearing, and then working with patients to achieve rehabilitation goals tosupport discharges home.

- Registered health care professional (HCPC registration ) / minimum of 3 years of experience in health improvement environment.) Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Specialist qualifications relevant to area of practice

- PRINCE 2 or equiv project management training PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study Working towards Master level study
