Job summary

Are you an experienced community support worker or community health care assistant looking for a new challenge as a temporary worker for the NHS and Kent County Council Home First Team in Kent?

Main duties of the job

'Home First' An approach whereby supporting people to be discharged home is the default discharge pathway, with alternative pathways for people who cannot go straight home. This principle is a key part of the discharge to assess model. The Home First Team service in Thanet, East Kent is a Kent Community Health NHS Foundation Trust (KCHFT) and Kent County Council (KCC) adult social care partnership. The team provides short-term, personal care and rehabilitation to support patients to return home following a hospital admission and facilitate early supported discharge from hospital through access to health and care needs assessment at home.
