We have an exciting opportunity for a Advanced Clinical Practitioner to join our caring and progressive team in Herne Bay.
You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care of people living with frailty within a variety of healthcare settings.
They will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the person living with frailty who may be acutely unwell, recovering from illness or injury or in need of proactive assessment.
You'll lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.
Main duties of the job
Autonomous practice: The role requires an advanced level of responsibility and autonomy to make professionally accountable decisions, including differential diagnosis, prescribing medication and delivery of care, often from referral to discharge in unpredictable situations. They are empowered to use their advanced knowledge, skills and judgement for high-level and complex decision-making in an expanded scope of practice role.
Critical thinking: Practising autonomously requires self-regulatory judgement that results in demonstrating the ability to interpret, analyse, evaluate and infer. Critical thinking enables ACP's to reflectively and rationally analyse evidence, cases and situations in clinical practice, enabling an advanced level of judgement and decision-making.
Advanced levels of decision-making and problem solving: ACPs will demonstrate expertise in complex decision-making in relation to their role. This includes determining what to include in the decision-making process, and making-a-decision, based on judgement and critical thinking/problem solving.
Values-based care: Individuals are required to have an advanced level of awareness of their own values and beliefs. Care is negotiated with service user/carers as an equal partner. Practitioners will consistently demonstrate 'working in a positive and constructive way with difference and diversity; putting the values, views and understanding of individual patients and carers at the centre of everything they do.
