Job summary
Edenbridge Memorial Health Centre is an exciting and ambitious new integratedservice, delivered in partnership by Kent Community Health Foundation Trust incollaboration with Edenbridge Medical Practice.The service will be delivered from a new purpose built unit; focussing upon theneeds of the local population, providing streamlined pathways to optimise healthand wellbeing of Edenbridge residents. The services incorporate:
- a GP service
- a Wellbeing Day Centre for people with frailty, dementia or health impacts ofisolation
- Outpatient services
- Wound clinic
- Proactive frailty services
- Same day urgent care service.
Main duties of the job
