We are looking for anenthusiastic and adaptable Health and Rehabilitation Care Assistantto join ourcaring and progressive team in Westbrook House, Thanet.

You will work as part of a close-knit team who are dedicated to the health and wellbeing of people in Thanet. Thanet is made up of a diverse patient group with complex needs. Healthcare assistants make a difference by helping to keep people at home and independent for longer.

Our community nursing teams are closely aligned with local Primary Care Networks, to make sure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.

Main duties of the job

In this role you'll provide clinical, health, personal care and rehabilitation for patients under the direction of the nurse in charge and senior therapists. The role is key to providing a 24/7 rehabilitation culture on theward where patients are encouraged and supported to practice delegated therapy tasks and daily living activities that support their recovery.

The overall aim is to support patients to maximise their functionalability and reduce to a minimum the amount of care they will need on discharge from the community hospital.

You will be expected to participate in 24-hour internal rotation which includes working a mixture of the following shifts: early, late, night and long days and undertake other duties where requested which arecompatible with the grading of the post.

You will be expected to move to other Community Hospital sites to cover the needs of the service.

Additional Working Pattern: Mix of early, late and night shifts over a 7 day rota

  • Relevant level 3 qualification (NVQ / QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent
  • Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)

Experience

Essential

  • Minimum of 2 years' experience as a Healthcare Assistant / Worker in a health or social care setting
  • Experience of working as part of a team
  • NHS Experience/UK work experience
  • Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster / TAPS RIO)

Desirable

  • Experience within community healthcare setting
