We are looking for an enthusiastic and adaptable Community Nurse to join our caring and progressive team in Hawkhurst Community Hospital as Team Leader.

We are a skilled team of healthcare professionals who provide nursing support for patients aged 18 and over in their home, delivering personalised care to those with complex and long term conditions so that patients can stay safe and well at home or in their chosen environment.

We operate seven days a week from 8am - 6pm, which allows us to accommodate flexible working hours for anyone who wishes to change their working pattern.

Weekend working is generally on a 1:4 ratio with days off during the week.

You will be required to drive your own car and are able to claim mileage costs and access the NHS car lease provision, following your probationary period.

Main duties of the job

There are two teams supporting patients registered with GP practices working in the Hawkhurst area and surrounding villages within the Weald Primary Care Network.

Each team has two senior nurses who take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff.

Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.

If you're an acute nurse with advanced clinical skills, this is a great opportunity to make the move over to community. KCHFT is keen to support development and therefore, if you don't have the specialist District Nurse qualification or a recognised advanced clinical skills/module, we will still be able to offer you a trainee position whilst we support you to gain the qualification. Additional Working Pattern: Monday to Friday between 8am and 5:30pm with rotational weekends
