Job summary

We are a large, nurse led Complex Care Team who provide support to long term condition and frailty patients in the community. Wemanage patient care both proactively and responsively through education, enablement and long-term condition management; keeping ourpatients living well, in their own homes to avoid unnecessary admissions. The service supports innovative practice and currently utilisesdigital remote reporting within a care homes project.This role will suit those looking to move from a primary care or acute setting to a well-developed and respected, supportive communityservice. Equally, the role is ideal for those looking to further develop their pre-existing community based knowledge and skills in long termcondition and frailty management.The team consists of Advanced Clinical Practitioners, Band 7 Complex Care Nurses and Advanced Healthcare Assistants. Due to the widerange of experience, clinical backgrounds, skills and knowledge we are very well placed to offer training posts to those wishing to move tothis area of clinical practice. As such we support a number of trainee roles both at trainee ACP and trainee Complex Care Nurse levels.This support can take the form of development at degree or Masters level in addition to stand alone modules including Enhanced ClinicalAssessment Skills and Prescribing

Main duties of the job

During the day you will be expected to visits patient in their own homes and take time with them to assess and review, devise bespoketreatment plans and work collaboratively with patients and health and social care partners to optimise patient management.The team has a strong MDT ethos and well-developed relationships in a number of sectors locally, as a result you will have access tospecialist and acute MDTs for escalation and support and work closely with other colleagues from urgent care and community nursing forexample.This is an operation lead role, so working in partnership with a colleague you will be assisting the operational manager with day-to-dayline management of colleagues and taking an active lead in some elements of team organisation.If you feel that this role suits you and you wish to discuss it further or arrange an informal visit please contact Ann-Marie Jethwa,Operational Manager on 07765897726 or ann-marie.jethwa@nhs.net
