We are a dynamic and forward-thinking Urgent Care service committed to delivering high quality, patient centred care. Our multi-disciplinary team thrives in a fast-paced environment, responding to a wide range of acute presentations with compassion, expertise and efficiency.
You will play a pivotal role in assessing, diagnosing and managing patients with minor injuries and illnesses, working autonomously and collaboratively to ensure timely and effective care.
We are seeking a clinically astute, motivated, and enthusiastic Advanced Clinical Practitioner to join our well established, experienced and friendly team. You will work to deliver timely and quality care, working closely with the Lead Nurses, medical and nursing team to manage and provide exceptional leadership and clinical guidance. This is a valuable opportunity for an Advanced Clinical Practitioner to work in an established but ever evolving service in which they will further develop and practice their nursing, clinical and leadership skills and contribute to its development.

Main duties of the job
Main duties:
- To work autonomously to assess, diagnose and treat patients presenting with urgent care needs.
- Prescribe medications and initiate appropriate investigations.
- Work closely with Nurses, GPs and other healthcare professionals.
- Support the service to provide care in line with defined performance indicators including quality standards, quality goals and patient experience.
- Contribute to service development and clinical governance.
- Provide leadership and mentorship to junior staff.
- To assess, plan, implement and evaluate programmes of evidence-based nursing & clinical care.
- To provide expert advice and clinical practice.
- To participate in the education and development of self and other staff.
- To work collaboratively and co-operatively with clinical colleagues to develop services and quality of care delivered.

Essential
- Relevant HCP qualification (e.g RGN/HCPC)
- Masters in advanced practice, or equivalent experience
- Independent Prescriber
- Experience of working in an Urgent Care/Community setting in either the in or out of hours period
Desirable
- Experience in end-of-life care
Experience and knowledge

- Aware of the importance of, and application of evidence-based practice
- Experience in managing primary care cases independently
- Evidence of relevant continuing professional development
- Experience in clinical audit and evaluation of cases and risk assessment
