Parkinson's Disease Nurse Specialist/Clinical Lead

Job summary

Are you a highly skilled and autonomous nurse passionate about providing expert care for patients with Parkinson's Disease? Join our dedicated team and make a real difference in the lives of patients and their families across the community.

We are seeking an experienced, motivated and autonomous practitioner to join our specialist nursing service for people living with Parkinson's Disease and related conditions.

In this highly specialist role, you will independently manage a small caseload, deliver expert clinical assessment, provide education and support to patients and their families, and lead on the development of evidence-based practice.

You will act as a key point of contact for patients across community settings--clinics, care homes and patients' own homes--ensuring they receive timely interventions, holistic assessment, safe medication management and compassionate care.

This is an exciting opportunity to help shape and develop services for people with Parkinson's Disease across Wirral, working closely with multidisciplinary and multi-agency partners.

Main duties of the job

As a Specialist Parkinson's Disease Nurse, you will:

  • Deliver expert, evidence-based nursing care and support to patients, carers, and families.
  • Manage a caseload of patients, providing comprehensive assessment, monitoring symptoms, and adjusting medications.
  • Lead nurse-led clinics, home visits, and care home support, ensuring continuity of care.
  • Collaborate with multidisciplinary teams, social care services, and voluntary organisations to provide seamless, patient-centred care.
  • Educate and empower patients to engage actively in their care, maximising independence and quality of life.
