Job summary

A Nursing Associate Role has become available across the Urgent Community Response (UCR) and Frailty Virtual Ward Teams.

This is an exciting opportunity for a candidate to work within an expanding team supporting people being supported to remain in their own home during crisis or periods of illness.

Urgent Community Response and frailty virtual ward sits within the Community Integrated Response Team (CIRT) based at St Catherine's Health Centre and Arrowe Park Hospital, within the Community Response Division.

Main duties of the job

To support the UCR and frailty ward MDT and be delegated tasks from nursing and community paramedic colleagues. To assess and support with treatment and review of acutely unwell patients in people's own environments, including but not exclusively, medications management, wound and pressure care; phlebotomy, catheter care, clinical observations, supporting health care assistants within the team; being part of in service training programmes and supporting audit.

  • Registered Nursing Associate on NMC register
  • Nursing Associate Foundation Degree
  • GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent.

Experience and knowledge

Essential

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Understands and acts in line with NMC professional standards for practice contained within Standards of Proficiency for Nursing Associates.
  • Understand requirements for NMC professional Revalidation
  • Knowledge of a broad range of conditions experienced by patients in community and general practice settings. This will include Long Term Conditions, and common mental illnesses, frailty and palliative and end of life care.
  • An understanding of the presentations of multiple pathology, depression and anxiety states and frailty, predominantly in older people.
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Understand the importance of following procedures and treatment plans
  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
