A new and exciting opportunity has arisen. We are seeking a Band 5 Public Health Nurse to join our fantastic team within a Centralised Contact Duty Hub. A part time post is available in various locations across Cheshire East, Knowsley, St Helens and Wirral - permanent position. This position would suit anyone wishing to work agile or remotely.

Main duties of the job

We currently deliver outstanding0-19 and 0-19+ and 0-25 Health and Wellbeing Servicesacross Wirral, Cheshire East and St Helens and Knowsley supporting over 400,000 children and young people. Our dedicated, innovative and forward thinking children and young people's services have a positive impact on the health and wellbeing of families in our communities.

We work closely with our commissioners and partners to deliver the best possible services for the children, young people and families.

About us

You will be joining a proactive skill mixed team working together to deliver the Healthy Child Programme . As part of the 0-19+ / 0-25 Services which with other local statutory, health and voluntary agencies to provideincludes Health Visiting and School nursing. You will be delivering public health advice and person centred packages of care via virtual contacts.You will be working in partnership with Local Authorities and other stakeholders across Cheshire East, Wirral, St Helens and Knowsley to take the local children's agenda forward.

  • Evidence of recent professional development

  • Mentorship Level 6
