Job responsibilities

4.1 Clinical Responsibilities

1. Understands and acts in line with NMC professional standards for practice contained within The Standards of Proficiency for Nursing Associates.

2. Understand when to seek appropriate advice to manage a risk and avoid compromising quality of care and health outcomes acting in line with local and national organisational frameworks, legislation and regulations to report risks, and implement actions as instructed

3. Uses a range of assessment tools pertinent to the persons needs to inform the assessment and assess risk for both patients and staff.

4. Demonstrate the ability to effectively and responsibly access, input, and apply information and data using a range of methods including digital technologies and share appropriately within interdisciplinary teams.

5. Demonstrate an ability to prioritise and manage their own workload and recognise where elements of care can safely be delegated to other colleagues, carers and family members.

6. Contributes to the provision of care for people with a diagnosis of heart failure and additional complex needs.

7. Exercise professional accountability in ensuring the safe administration of medicines in line with Trust policies to those receiving care.

8. Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns to Caseload Manager and Clinical Team Leader

1. Undertake safe storage, and disposal of medicinal products in line with Trust policies.

10. Is able to show an awareness of the roles, responsibilities and scope of practice of different members of the nursing and interdisciplinary team, and their own role within it

11. 11. Assume responsibility for monitoring the progress of all patients undergoing cardiovascular rehabilitation evaluating their condition and adapting their programme as necessary within department standards and protocols seeking senior assistance as required

1. Collaborating effectively with other members of the multidisciplinary team or other agencies involved in the persons care.
   1. Recognise and apply knowledge of commonly encountered mental, physical, behavioural and cognitive health conditions when delivering care. Additionally demonstrate and apply an understanding of how and when to escalate to the Caseload Manager.
   1. support, supervise and act as a role model to nursing associate students, health care support workers and those new to care roles, review the quality of the care they provide, promoting reflection and providing constructive feedback.
   1. Work in partnership with people, to encourage shared decision making, in order to support individuals, their families and carers to manage their own care when appropriate. Whilst minimising risk for both people and staff, making use of all available resources.
   1. Promotes and applies the aims and principles of health promotion, protection and improvement and the prevention of ill health when engaging with people
   1. Protect health through understanding and applying the principles of infection prevention and control, including communicable disease surveillance and antimicrobial stewardship and resistance.
   1. Assist in the deployment and organisation of the intermediate phase of cardiac rehabilitation to include educational talks, exercise and relaxation classes
   1. Ensure adequate levels of supervision of all rehabilitation sessions in line with service policies and procedures and according to the patients care plan
   1. As part of promoting a safety culture recognise and report via Datix, any situations, behaviours or errors that could result in poor care outcomes
   1. Is involved in audits and distributes and encourages completion of patient experience questionnaires.
   1. Maintains good working relationships with colleagues and other agencies. Understand the demands of professional practice and demonstrate how to recognise signs of vulnerability in themselves or their colleagues and the action required to minimise risks to health

   11. Is required to communicate effectively with patients, carers, relatives and other professionals

   4.2 Professional Responsibility

   1. Recognises personal accountability and responsibility to monitor and evaluate care to ensure optimal practice

   2. Understands and works within NMC professional standards for practice contained within The Standards of Proficiency for Nursing Associates, maintaining own professional development and competence to practice. Maintaining revalidation and registration.

   3. Take responsibility for continuous self-reflection, seeking and responding to support and feedback to develop professional knowledge and skills

   4. Provide, promote, and where appropriate advocate for, non-discriminatory, person-centred and sensitive care at all times. Reflect on peoples values and beliefs, diverse backgrounds, cultural characteristics, language requirements, needs and preferences, taking account of any need for adjustments

   1. Has read and works in accordance with current Trust policies, procedures and clinical guidelines.
   1. Ensures own mileage form and extra duty forms are submitted to the Clinical Team Leader within the agreed time frame.
   1. Engagement with appraisal and the development and activation of a personal development plan.
   1. Attends mandatory training and other clinical training essential to the role. This includes undertaking e- learning.

Person Specification

Qualifications and Professional Training

Essential

• Nursing Associate Foundation Degree or equivalent training/experience
• GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent.
• Basic Life Support

Desirable

• Registered Nursing Associate on NMC register
• Electrocardiogram competencies / training

Experience of Knowledge

Essential

• Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
• Understands and acts in line with NMC professional standards for practice contained within Standards of Proficiency for Nursing Associates.
• Have a level of knowledge of cardiovascular rehabilitation and clinical knowledge of associated treatments, interventions and outcomes along with basic knowledge of a broad range of conditions experienced by patients in community and general practice settings. This will include Long Term Conditions, and common mental illnesses, frailty
• Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
• Understand the importance of following procedures, treatment plans and exercise programmes
• Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
• Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
• Experience of providing and receiving complex, sensitive information

Desirable

• Experience of working within the community setting
• Understand requirements for NMC professional Revalidation
• Up to date knowledge of national and local strategies to reduce CVD

Skills and Attributes

Essential

• Ability to take part in reflective practice and clinical supervision activities
• Ability to organise and prioritise own delegated workload
• Ability to communicate effectively (written, verbal and nonverbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.

• Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting.

Desirable

• Ability to use IT systems Systm 1, Cerner
• Ability to work unsupervised with team support and as a lone worker
