Job summary

Within a multi-disciplinary environment, assist in the delivery of a comprehensive programme of secondary/primary prevention of cardiovascular disease and other cardiac diagnoses. The programme will be delivered within the Wirral Heart Centres across the St Catherine's Health Centre, Victoria Central Health Centre and West Kirby Marine Lake sites. The post will require performing ECGs, taking basic measurements such as height, weight and blood pressure. Excellent communication skills and an ability to work in a team and to direction is essential. Using initiative and having excellent organisation skills is key. Team work is a major feature of this role and the ability to work and communicate with a diverse and multidisciplinary team.

Main duties of the job

4.1 Clinical Responsibilities

1. Understands and acts in line with NMC professional standards for practice contained within The Standards of Proficiency for Nursing Associates.

2. Understand when to seek appropriate advice to manage a risk and avoid compromising quality of care and health outcomes acting in line with local and national organisational frameworks, legislation and regulations to report risks, and implement actions as instructed

3. Uses a range of assessment tools pertinent to the persons needs to inform the assessment and assess risk for both patients and staff.

4. Demonstrate the ability to effectively and responsibly access, input, and apply information and data using a range of methods including digital technologies and share appropriately within interdisciplinary teams.

5. Demonstrate an ability to prioritise and manage their own workload and recognise where elements of care can safely be delegated to other colleagues, carers and family members.

6. Contributes to the provision of care for people with a diagnosis of heart failure and additional complex needs.

7. Exercise professional accountability in ensuring the safe administration of medicines in line with Trust policies to those receiving care.

8. Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns to Caseload Manager and Clinical Team Leader

  1. Undertake safe storage, and disposal of medicinal products in line with Trust policies.

10. Is able to show an awareness of the roles, responsibilities and scope of practice of different members of the nursing and interdisciplinary team, and their own role within it

    1. Recognise and apply knowledge of commonly encountered mental, physical, behavioural and cognitive health conditions when delivering care. Additionally demonstrate and apply an understanding of how and when to escalate to the Caseload Manager.

  • Nursing Associate Foundation Degree or equivalent training/experience
  • GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent.
