Job summary

We are recruiting to a team of dedicated community nursing staff to work with ICCT Service and our Primary Care Networks. This exciting opportunity means that you'll be working as part of a combined team of community care staff and PCN colleagues to proactively support people with moderate and severe frailty as part of WCHC's Population Health Management.

You will be based within the ICCT service working 22.5 hours per week.

This is a permanent position.

Main duties of the job

This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. You will be working in both lone capacity and in conjunction with other team members, ensuring clinical standards are maintained using evidence based practice. You will have a good knowledge of the components of Clinical Governance, Best Practice, Risk Management, Clinical Audit and relevant Professional Codes. You will be able to evidence excellent clinical skills.

Qualifications and Professional Training

Essential

  • Registered General Nurse educated to degree level (or equivalent)
  • Nurse Prescriber or willing to undertake
  • Specialist Practitioner Qualification - District Nursing (or equivalent postgraduate modules, nursing qualification/ experience
  • Care of the Older Person or similar course (or willing to undertake)
  • Evidence of further recent study/ professional development
  • Leadership qualification - ILM level 6 or equivalent (or willing to work towards) or relevant equivalent experience; mentorship

Desirable

  • Minimum 3 years post registration experience

Experience and Knowledge

Essential

  • Knowledge/experience of working with older people
  • Knowledge of vulnerable adult's procedures and safeguarding
  • Community Nursing experience
  • Experience of clinical assessment, analysis, planning, delivery and evaluation skills
  • Experience of managing complex and challenging cases
  • An excellent understanding of Frailty and applying this to practice
