Early Intervention Assistant

Are you a Health Care Assistant, Clinical Support Worker or Early Intervention Assistant who wants to shape the future of nursing care for people with moderate and severe frailty in Wirral?

We are recruiting to a team of dedicated community nursing staff to work with Primary Care Networks. This exciting opportunity means that, you'll be working as part of a combined team of community care staff and PCN staff, proactively supporting people with moderate and severe frailty as part of WCHC's Population Health Management.

You will be based in ICCT working 37.5 hours per week.

This is a permanent post.

Main duties of the job

As part of this team, you will be actively working to ensure we recognise those people who will benefit from person-centred and holistic assessment and care planning, and then receive expert, compassionate, well-coordinated support. (This is often described as the process of Comprehensive Geriatric Assessment, or CGA.)
