Job summary
A senior heart failure nurse position is now available within the exciting and forward thinking community heart failure team. We offer care to patients on the Wirral with a confirmed diagnosis of heart failure. This includes complex care management and the role can be challenging and rewarding.
This post will support the expanded team and allow for an experienced and clinical expert in heart failure nursing care to join the team on developing the joint working partnership between primary, secondary and community care to improve heart failure services across Wirral
The team is dynamic and all staff require a positive attitude to change, enhanced communicational skills and the ability to liaise directly with allied health professionals.
The role will require the applicant to have all the essential criteria as per personal specification.
Main duties of the job
Be responsible for the efficient and effective delivery of a co-ordinated community based heart failure service for patients in Wirral.
Assessment and evaluation of patients with a diagnosis of heart failure Clinical decision making and collaboration with the community health care team are essential.
Provide expert, specialist heart failure care advice and education to clinical colleagues (multidisciplinary team) and patients/carers/relatives
Provide leadership in the field of cardiology and draw on advanced knowledge and exercise professional skills to develop and deliver high quality service to people with heart failure.
Line management responsibility for the Band 6 practitioners in the community heart failure team inclusive of professional development, management supervision, managing attendance, performance management, recruitment and competencies using the Trust appraisal process and policy, authorising annual leave and Sickness monitoring.
Lead the development, delivery and evaluation of clinical, educational, strategic and organisational heart failure services throughout Wirral CT. The role involves providing specialist heart failure nursing input and leadership for all nurses involved in the care of patients with heart failure care needs across the Trust and influencing the development of the whole service to ensure it is patient-centred, safe and efficient and in-line with the appropriate standards.

Use of digital technology such as Carelink and Telehealth as a means to assess patient need for review and make changes to treatment and/or care
Initiate and optimise patient management through evidence based pharmacological and non-pharmacological interventions.
Undertake the duties and responsibilities of the Heart Failure Nurse within a professional code of practice and the Skills for Health CHD Competency Framework.
Perform extended roles as required, ECG recordings, venepuncture
Communicate highly complex information relating to diagnosis, prognosis and treatment to patients and their relatives/carers plus multidisciplinary colleagues in Primary and Secondary Care
Organise own caseload and clinics in the community setting/ward visits at WUTH without supervision utilising good time management skills
Support the provision of specialist expert advice on issues relating to heart failure to patients, carers, primary health care teams, nursing and residential home staff, allied health professionals, social services and other people involved in the provision of care
Provide regular supervision and performance management for B6 team members
To act as a role model for other nurses, caring for patients with heart failure care needs establishing positive and credible professional leadership and be involved in mentoring of staff and clinical supervision as appropriate
