Job summary
We are seeking an enthusiastic and motivated Band 3 Healthcare Assistant to join our Community First (D2A) team in Derby City.
Previous experience of working in health and social care is essential
As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.
A good standard of English and Maths (GCSE or equivalent) and NVQ Level 3 (or equivalent) is required, along with excellent communication skills.
Access to and ability to use transport within the Derby and Derbyshire locality is essential. You will work as part of a wider integrated team providing holistic care to patients in their own homes and care home settings.Community HCA's carry out a plethora of patient care including wound care, catheter care and administration of injections.

Main duties of the job
This post will involve working within the Community First D2A team visiting patients within their own homes.
We also have patients at Perth House - a facility designed to support patients stepping 'down' from hospital with a period of further assessment and also, in stepping 'up' from the community, in order to prevent unnecessary hospital admission.
You will follow prescribed care and implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
To provide the delivery of high quality care and actively support the D2A nursing team in the management of the health care environment.
To implement the relevant service.
To work in close collaboration with the DAU, UCR, and Integrated Community Teams.
