Main duties of the job
To provide specialist Occupational Therapy assessment and treatment for adults as part of the Early Supported Stroke Discharge team predominantly across Amber Valley. There may be occasion where the post holder is asked to support the team in Derby City or South Derbyshire where this is practicable and capacity allows.
To work as a member of the multi disciplinary team, working closely with therapists, support workers and nurses.
To educate and train other Occupational Therapy staff, healthcare professionals, students, support workers and carers.
To participate in the development of effective, responsive, evidence based and patient centred services.
To assist on specific projects in agreement with the Team Leader and Operational Lead for the Service.

To undertake specialised, detailed, complex and accurate assessments of patients on Early Supported Stroke Discharge Pathway (ESSD) and identify and carry out appropriate Occupational Therapy interventions in conjunction with multidisciplinary team aims and individual goal setting.
To actively participate in multi disciplinary treatment planning and goal setting to facilitate collaborative care and a high-quality service.
To utilise clinical reasoning skills in order to make appropriate assessments, treatment planning and interventions.
To prioritise and manage own Occupational therapy caseload, while analysing and interpreting highly complex and sensitive information to set goals with patients relevant to their needs.
To have effective knowledge of stroke and impact on function. To be able to formulate individual treatment planning and goal setting using appropriate models of practice and where the condition may require further investigation / treatment refer to relevant professional / services.
To have substantial knowledge and understanding of psychological and psycho-social, cognitive and behavioural factors and their impact on function; and to be able to assess and take into account when planning treatment and goals using standardised tools and assessments.
