The Discharge Improvement Lead is a key system leadership role working collaboratively with senior colleagues across health and social care and the voluntary sector. The aim is to deliver the 'Discharge Improvement Strategy' for Joined Up Care Derbyshire (JUCD). Our strategy starts with the question 'Why not home, why not today?' which all our partners across health and social care have signed up to. By working together, we are committed to helping as many people as possible return to their home after a stay in hospital, in line with the national Home First principles.

Main duties of the job
This is a system role, as opposed to organisational role, so you will be tasked with delivering a programme of work without the control of resources. Instead, you will work collaboratively, with all system partners, to agree priorities of transformation and ultimately delivery of these priorities.
Working with partners, you will lead the process to enable decision making regarding options for improvement, this will be objective, evidence based and benchmark against best practice from other systems. The job will require matrix working, strong influencing and negotiating skills with the ability to form trusted relationships at all levels of the system. You will also be required to have highly developed organisational skills, with a proven history of leading change across organisational boundaries. You will be seen as a trusted, and impartial role model.
The post holder will champion improvement. Engaging with and motivating colleagues to generate and implement ideas to nurture an improvement culture.
The post holder will effectively communicate with all system stakeholders at varying levels of expertise and responsibility.
About us
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
