Job summary

A rare opportunity has arisen for a Advance Support Worker to join the Team Up Home visiting Service in North Hardwick and Bolsover PCN . To work as part of a small team of Community General Practitioners and ACPs to support multi-disciplinary teams for North Hardwick and Bolsover PCN. This is direct work with patients and their carers either in person, however the main remit is to support the clinical practice and development of the Team Up service working with other clinicians. To assist with and perform the provision of holistic, high quality, evidenced based health care to a defined local population, working within an appropriate supervisory and monitoring framework, to an agreed, multi-disciplinary treatment plan, by providing ongoing treatment, teaching and instruction to their own referred caseload of patients. The post holder will organise and prioritise their own day to day work tasks and activities, and delegate tasks to junior colleagues. To attend all required mandatory and specific to role training

Main duties of the job

To manage a delegated caseload to provide holistic care to patients in their own homes or alternative health care settingto promote recovery and encourage and maintain independence.To communicate information to patients and carers regarding their health needs and associated treatment plans.To report and take appropriate action regarding any change in patient condition or circumstance and communicateverbal and written information to all relevant integrated care team membersTo maintain and record accurate health records in line with DCHS/ Team Up policies and procedures and Calldicott requirements
