We are a looking for qualified nurses to join our Matlock District Nursing Team to complement our current team. The Matlock team is for a full time position of 37.5hrs based at The Whitworth Hospital.

This is a great opportunities to join one of our teams in the Derbyshire Dales.

Main duties of the job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
