DCHS Children's Services are commissioned to deliver an Integrated Specialist Intensive Home Visiting Service (Intensive Health Visiting), and currently have an exciting opportunity to offer to an experienced SCPHN Health Visitor to come and be a part of the Intensive Health Visiting Team within 0-19 Children's Services.

We are seeking to appoint a dedicated and motivated Health Visitor to work within the Amber Valley areas, on a part time basis.

Main duties of the job

Delivery of an intensive home visiting service to vulnerable women, children and families.

Development of therapeutic relationships with clients.

Working intensively within complex family situations providing family focusedevidence-basedcare.

Provision of services in collaboration with partner agencies, statutory organisations and the voluntary sector, working together to improve service provision and outcomes for this population group, both within your locality and across the service.

Excellent communication skills are essential to effectively engage with your client group, peers and partner agencies, leading by example and promoting a supportive and caring culture within your team.

Working independently and together with other members of the ISIHVS team, along with colleagues within 0-5 Health Visiting Services, to reach optimal outcomes for your client population.

This demanding role requires high levels of professional skills, clinical judgment, and practice.You must be a competent and experienced safeguarding practitioner.

Responsible for delivering the ISIHVS within Derbyshire to women and families who have identified vulnerabilities, from the antenatal period - 2.5 years.

Delivery of the HCPat universal partnership level and above, following the enhanced service pathway, along with progressive levels of service according to identified need.

Responsible for the delivery of the services key performance indicators for your identified client group.
