Job summary

We currently have an exciting opportunity for highly motivated and enthusiastic ACP within our TeamUp Erewash Frailty services.

The service is managed through Erewash Health Partnership, although you will be employed through DCHS. You will based at Long Eaton Health Centre, and may be asked to work across the Erewash region. You will be required to work in Residential or Care homes offering a first class care and provide clinical care for housebound patients that may be frail and elderly. You will be supported by a community GP and additional qualified nursing staff within the team. You will have a UK driving licence and access to your own vehicle for work purposes.

Are you an existing or an aspiring Advanced Clinical Practitioner with experience of community care?

A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment.

Main duties of the job

The successful candidate will deliver first class clinical care to the frail and elderly people in Erewash. This maybe during a routine ward round at a residential or nursing home, or directly at a patients home as part of our frail acute service.

As an ACP you will:

  • Provide clinical leadership, including direct clinical care, and advice on complex issues
  • Establish a cohesive team approach for the ACP workforce development
  • Support the TeamUp service leads and managers with audit, innovation, and performance management
  • Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
  • Assist in feedback for staff appraisals.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults.
