Job summary
We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) or a trainee Enhanced Clinical Practitioner to work in the Amber Valley Health visiting service.
TheHome Visiting service works collaboratively with the local GP and Integrated Teams in Amber Valley which provides a varied source of support. Our patient group is patients in their own homes.
Our team is skilled and provide an excellent working environment with opportunities to learn and develop in this role.
You will improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, or prevent inappropriate hospital admission. We will positively consider applicants who fulfil the Person Specification, with the exception of non-medical prescribing and advanced clinical decision making/patient examination skills.
However, we will consider those applicants who do not meet this criteria and are willing to accept a Band 6 Trainee Enhanced Practitioner role until successful completion of both programmes and the completion of a training portfolio.
The role will be either Band 6 or Band 7 (dependent on experience and qualifications).
Main duties of the job
The team provide a holistic approach to patients in their own homes, delivering health education/ promotion, proactive clinical assessments, care planning, annual assessments and long term conditions management.
You will work as a key member of the multi-disciplinary team, working alongside General Practitioners, Community Pharmacists, Care Co-ordinators, Community Psychiatric Team, Physiotherapists, Occupational Therapists and Social care providing joined up care.
