Main duties of the job
Provide high quality rapid nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.
Implement/refer to the relevant services as appropriate.
Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.
