Our Community First Team has opportunities for Health & Social Care Assistants to join our team in Derby City.
What does the Community First Team do?
We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The person needs short term/interim support in the Perth House Assessment Hub. The teams are committed to caring and supporting people, to enable them to maximise their independence.

What are the hours?
You will be working a 4 on 4 off rota pattern, consisting of morning shifts7.15am - 14.45pm and evening shifts 14.30pm - 22.00pm
The rota pattern will include weekends and bank holidays.
Where would I be working?
Base: Perth House Assessment Hub, Athlone Close, Chaddesden, Derby. DE21 4BP.
Main duties of the job
The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual care without direct supervision in the Perth House Assessment Hub.
To provide support to patients to optimise their health and independence.
