EARLY SUPPORTED STROKE DISCHARGE DERBY CITY
An exciting secondment opportunity is available to work within the Derby City ESSD team as a Band 7 Occupational Therapist from January 2025 until end December 2025.
This role will work as an integral part of the multi-disciplinary ESSD team in Derby City supporting people to be discharged from hospital following stroke, delivering goal centred rehabilitation and support in line with national clinical guidelines. The team contribute to SSNAP data and liaise closely with local and regional colleagues to share good practice.
The Derby City team is one of 3 teams providing an ESSD service in the South of Derbyshire, the teams work in collaboration with each other and support capacity where necessary across all 3 areas.
You will bring experience in stroke rehabilitation, with specialist skills to independently carry a complex caseload as well as provide guidance to Band 6 OTs and other colleagues within the team. In addition they will be undertaking the leadership and implementation of a service improvement project.
Main duties of the job
To provide effective and evidenced based clinical and professional leadership to the Early Supported Stroke Discharge Team and the Stroke and Neurology service as a whole.
- To provide highly specialist Occupational Therapy intervention to patients within the team.
- To act as a source of advanced knowledge in the assessment and treatment of Stroke.
- To lead and implement a service improvement project within the scope of the secondment.
- To provide clinical leadership within the team and service as a whole.
- To provide education, training and support to other Occupational Therapists, students and members of the MDT.
- To lead on and support service development.
- To provide assessment and treatment using face to face, telephone and virtual consultations.
